Disability Reports and Forms
Those individuals pursuing long-term disability on a psychiatric basis and who have been out of work because of psychological conditions on the job site may need disability certifications or evaluations.

Letters
Letters can be written on behalf of patients. When applicable, the charge for letters will be based on the complexity and time involved to complete the letters. A request for such a letter will require a signed release form.

  1. You have the right to receive information about our services and qualifications, clinical guidelines, and patients’ rights and responsibilities.
  2. You have the right to be treated with respect and recognition of your dignity and need for privacy.
  3. You have the right to participate fully in decision-making regarding your treatment planning.
  4. You have the right to voice complaints or appeals about the care provided to you.
  5. You have the responsibility to provide, to the extent possible, information that we need in order to care for you. This includes all medication, alcohol and drug use. 
  6. You have the responsibility to follow the plans and instructions for care that you have agreed upon.
  7. You have the responsibility to participate, to the degree possible, in understanding your behavioral health problem(s) and developing mutually agreed upon treatment goals. 
  8. Mutual respect is fundamental to treatment. Aggressive or threatening behavior either physical, written or verbal will not be tolerated and will be grounds for terminating your treatment at my clinic. If you feel mistreated by staff or a provider, please let Dr. Deeves or the Clinic Manager know so they can address your concerns.

Our policies are designed with your well-being in mind, aiming to guarantee the utmost care and attention for every patient we serve.

Appointment times are available Monday, Tuesday, Thursday and Friday between the hours of 8:00 a.m. to 5:00 p.m. Wednesday appointments are available between 8:00 a.m. and 2:00 p.m. 

Please arrive on time for your appointments. We may need to reschedule the appointment if you arrive after the appointment time.

Patients are asked to cancel at least 24 hours in advance of scheduled appointment time. There will be a $50 late cancellation charge if you do not cancel your appointment within that time limit.

You will be charged a $100 No-Show Fee if you do not show up to your appointment.

We require a reservation fee of $100 to schedule your first appointment. If you have an in-network insurance, this fee may be refunded to you at your first appointment depending on your insurance plan/coverage.

You will then be charged for your visit in accordance with your insurance plan/coverage.

For new patients, we ask that you cancel your appointment no later than 3 business days before your schedule appointment by speaking to a staff member. Late cancellation of your appointment will result in your $100 reservation fee not being refunded.

THE PATIENT PORTAL IS NOT FOR EMERGENCIES OR REFILLS.  

The patient portal is the preferred place to complete clinical follow up forms for your provider which are called measures. The measures are to be completed before each follow up appointment and typically are sent to your patient portal 2 days prior to your appointment where you are notified via email.

All Refills must be initiated by the patient via a telephone call or by the REFILL REQUEST tab on this site. No other method is authorized. 

Prescriptions are generally written in a quantity to last until the next scheduled appointment. Please fill your  prescriptions promptly. Controlled substance prescriptions are valid for 21 days. If it becomes necessary for a refill to be called in due a missed appointment, a charge of $20.00 may be assessed. A charge of $20.00 will be applied to prescriptions and triplicate medications that are written between appointments. A $30.00 charge will be applied if a prescription must be rewritten due to the loss or expiration of that prescription. 

Requests for prescription refills are to be called into the office or sent via the REFILL REQUEST tab from Monday through Thursday for approval. A minimum of five working days is typically required for the prescription refill to be approved. Prescription refills cannot be ordered or approved after business hours because your chart may not be available. 

Controlled substance prescriptions that are stolen require an appointment and a police report to be replaced / refilled. Those may be sent to the clinic on the REFILL REQUEST tab or brought to your appointment. Stimulant prescriptions are not refilled outside of an appointment. 

Prior Authorizations may take up to seven days and a charge  of $25.00 may be assessed for this service. There is no guarantee the insurance companies will approve them.

To request copies of your medical record, a release information form must be signed. Your request will be fulfilled within 15 business days upon receipt of the medical release form.

Fees: Per rules adopted by the Texas State Board of Medical Examiners: $25.00 for the first 20 pages, $.50 cents for each page thereafter. No charge Doctor to Doctor/Hospital.

Charges will be assessed for letters and completion of forms.

Payment is expected at the time of your appointment. 

Our contracts and agreements with insurance companies and health plans require us to collect all copayments and deductible amounts at the time of service. If you feel there are extenuating circumstances surrounding the missed or late canceled appointment you may contact the billing department by calling the office or using online billing through our website. 

Billing statements are sent out monthly. If you are having an issue with paying your bill please contact our billing department right away. We care about your treatment and are willing to work with you on your balance.

Please Read the following important information about refill requests.

  • If you are due for a follow-up and need a refill,  you must schedule your appointment first.

  • Refills will be submitted to your pharmacy in a quantity that will allow you to attend your next scheduled appointment.

  • Lost or stolen controlled medications will require a police report in order to get a refill.
