Please note the following conditions must be met to schedule an appointment. 

• We are happy to share information on getting an appointment with friends, family, and loved ones, however the person who is going to be the patient must make the appointment. 
• A credit card must be kept on file to make an appointment.
• All required paperwork must be finished prior to the appointment. 

The intake appointment will last about  one hour and will be in person at our clinic.
